Visiting Team Information

Games will be hosted in the Hicks Student Center (Building H). The entrance is located on the southeast corner of the building, and the athletic training room is located just through the entrance on the right-hand side.

 

 

A certified athletic trainer is present at every home game. The athletic training room will be opened two and a half hours prior to every game and will remain open up to one hour after the game as needed.

Be sure to provide the team’s own taping supplies and a list of the required taping/wrapping techniques and treatment for each athlete. Athletes will have access to moist hot packs and can utilize electrical stimulation and ultrasounds treatments by a written request with parameters from their own athletic trainer.

Supplies that will be provided at the bench during each game include:

Water

Water cups or bottles

Bench towels

First aid kit

**Further assistance in first aid can be provided by the certified athletic trainer if desired

Life Pacific University coaching staff is trained in CPR and First Aid and is able to assist the certified athletic trainer if an emergency situation arises for either team if the visiting coaching staff is unable to do so. The athletic training room is equipped with spine board, vacuum splints, SAM splints, and an AED, which will be easily accessible at every home game.

Ice bags and wrap will be provided for the visiting athletes as needed. If any injuries occur during the game, the athletic trainer will document and forward the information to the team’s athletic trainer via email.

Mandatory Pre-Participation Paperwork

All student-athletes will receive an email in early May from the Head Athletic Trainer regarding how to create/login to our online injury database SportsWare and access their student-athlete portal. Student-athlete profile information and primary insurance information must be completed by May 31st. Life Pacific utilizes our athletics insurance policy Relation Insurance to go through an insurance verification process to be sure that all student-athletes primary insurance is, in fact, active, as it is a mandatory requirement. If a student-athlete does not have primary insurance, they should speak to our Head Athletic Trainer ASAP regarding primary insurance policy options through Relation Insurance or assistance in applying for Covered California. Inactive or non-existent primary insurance will result in restriction from LPU participation until it is resolved.

 

SPORTSWARE INSTRUCTIONS

 

COMPLETING INSURANCE TAB INSTRUCTIONS

 

The mandatory forms that are accessible on the student-athlete portal of SportsWare will be electronically signed and submitted through the portal. These forms must be completed prior to being checked in for on-campus Pre-Participation Physical Examinations. The following are the required forms on the portal to be electronically signed:

 

  • Waiver of Liability, Assumption of Risk, Permission to Treat
  • Understanding Insurance Waiver
  • Drug Testing Consent Form
  • HIPAA Authorization for Use and Disclosure

 

Pre-Participation Physical Examinations (PPEs) will be held on-campus with our team sports medicine physician. The cash price of each physical is $25.00 and is the financial responsibility of the student-athlete. If any student-athlete wishes to opt-out of this on-campus physical (that does not include the baseline concussion testing), they must sign and submit a Declination of LPU PPE form directly to the Head Athletic Trainer at dhaines@lifepacific.edu. This must be submitted prior to June 1st so that on-campus PPEs can be scheduled appropriately. The Physical Examination Form must be completed by a Medical Doctor (MD) or Doctor of Osteopathic Medicine (D.O.), not by a Doctor of Chiropractic, Nurse Practitioner (NP), or Physicians Assistant (PA).
